Job Summary

The Intake Specialist will play a vital role in ensuring a seamless patient experience at our health center. This individual will be responsible for orienting new patients to our services, conducting insurance verification, and facilitating enrollment in insurance plans as needed. Additionally, they will ensure that sliding fee scale status is up to date and accurate on a daily basis, conduct PRAPARE assessments, and assist the Patient Care Coordinator with medical case management as needed.

Key Responsibilities
  • Complete PRAPARE assessments for new patients and identify program eligibility for various services.
  • Track the status of patient referrals to internal case management programs.
  • Orient clients to health center services and ensure a smooth transition into care.
  • Complete sliding scale applications for uninsured/underinsured patients and upload required documents into the electronic medical record.
  • Ensure up-to-date status of sliding scale applications for patients on a daily and consistent basis.
  • Facilitate patient enrollment into insurance plans, including ADAP, via the Marketplace and assist with entitlements.
  • Assist clients with changing their insurance and document administrative interventions in the medical record.
  • Maximize the percentage of client alignment of their primary care provider with patients' health plans.
  • Participate in morning huddles, regular staff meetings, and other team-building activities.
  • Support and participate in advocacy efforts to end the twin crises of homelessness and AIDS.
  • Assist providers with client needs related to the Intake Specialist role.
  • Collaborate with front desk staff on matters related to insurance, sliding fee scale status, and registration on a day-to-day basis.
  • Assist with patient registration processes with professionalism and accuracy, as needed.
  • Verify insurance for every visit unless uninsured, in which case, facilitate accurate sliding fee scale applications and documentation.
  • Ensure front desk staff remain active on insurance portals and utilize them for every appropriate patient transaction.
Requirements
  • High School Diploma required; Associates Degree preferred.
  • Must complete certified assistor training within the Marketplace upon employment.
  • The candidate must have knowledge in HIV/AIDS and entitlements.
  • The candidate should be able to perform scripted assessments and demonstrate the ability to discern emergencies and alert the appropriate personnel for assistance and support.
  • Commitment to confidential and HIPAA regulations on requests for medical documents is required.
  • Strong organizational skills and attention to detail are necessary.
  • Skilled in interpersonal relationships with a culturally diverse population.
  • Proficiency in Spanish is preferred.
  • Candidate must be willing to work on weekends and evenings, with advance notice.
  • Must have experience with Microsoft Office (Word, Excel), Outlook, and the Internet.
  • Must possess a high level of interpersonal skills to handle sensitive and confidential client information and situations.
  • Must be willing to participate in advocacy events.
