Diabetes Education and Management Specialist

We are seeking a highly skilled and experienced Diabetes Education and Management Specialist to join our team at Optum. As a key member of our Diabetes Education team, you will work closely with our Endocrinology providers and Primary Care providers to provide comprehensive diabetes education and management services to our patients.

Key Responsibilities:

  • Provide one-on-one consultation for patients on diabetes devices and management plans
  • Develop and implement Diabetes Self-Management Education and Medical Nutrition Therapy programs
  • Teach diabetes classes and workshops to patients and their families
  • Document patient progress and outcomes in Epic
  • Collaborate with the diabetes education team to develop and implement a comprehensive diabetes device program
  • Provide education and support to patients and their families on diabetes management and nutrition
  • Attend monthly meetings for Certified Diabetes Care and Education Specialists

Requirements:

  • Current registration and certification as a Registered Dietician (RD) and Certified Diabetes Educator (CDE)
  • AHA-BLS certification
  • Experience in diabetes education and management, with a strong understanding of diabetes management and various diets for medical nutrition therapy
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, medical staff, and the public
  • Competent with Microsoft Office (Word, Excel, PowerPoint) and Epic

Preferred Qualifications:

  • Bachelor of Science or Master's degree in Nutrition or a related field
  • Certified Diabetes Care and Education Specialist (CDCES) certification
  • Device certification with Medtronic, Tandem, Omnipod
  • 2+ years of experience in diabetes self-management education, medical nutrition therapy, and diabetes management
