Job Description:
We are seeking a skilled and compassionate Registered Nurse (RN) to join our Home Health team as a Case Manager. In this role, you will play a critical part in providing holistic care to patients in need. You will assess patients' social and emotional needs, develop individualized care plans, and collaborate with healthcare professionals to achieve optimal outcomes. Your primary goal will be to promote patients' independence and self-management skills during their home health episode.
Responsibilities:
You will be responsible for supervising and managing all aspects of patient care, including LVN and HHA supervision, case conferences, and discharge planning. You will also coordinate the distribution of visits among all disciplines, initiate referrals to ancillary services, and facilitate interdisciplinary communication to ensure seamless patient care. Additionally, you will provide education and support to patients and families, complete necessary documentation, and maintain accurate records.
Requirements:
To succeed as a Case Manager at Cantex, you must possess a strong clinical background, excellent communication skills, and the ability to work independently. A minimum of 2 years of experience in Home Health is preferred, but not required. HomeCare HomeBase experience is also desirable. You should be able to read, write, and speak English fluently and have reliable transportation. Valid and current auto liability insurance is mandatory.
