Surgery Partners is seeking a skilled Medical Case Manager to join our team. As a Medical Case Manager, you will play a critical role in coordinating patient care and ensuring seamless transitions throughout the healthcare continuum.

About the Role

This is a full-time position that requires a high level of collaboration with physicians, nurses, and other healthcare professionals to develop and implement individualized care plans for patients. The ideal candidate will possess strong analytical and communication skills, with the ability to effectively navigate complex medical situations and coordinate resources to meet patient needs.

Responsibilities
  • Develop and Implement Care Plans: Collaborate with interdisciplinary teams to assess patient needs and create comprehensive care plans that address medical, social, and emotional requirements.
  • Coordinate Transitions of Care: Ensure smooth transfers between healthcare settings, including hospitalizations, outpatient services, and home care.
  • Manage Utilization Review: Perform ongoing assessments to ensure appropriateness of care and identify opportunities for cost savings without compromising quality.
  • Counsel Patients and Families: Provide education and support to patients and their loved ones, empowering them to make informed decisions about their care.
Requirements
  • Bachelor's degree in Nursing or related field.
  • Licensure as a Registered Nurse (RN) required.
  • Minimum 5-10 years of case management experience preferred.
  • Certification in Case Management (CCM) desirable.
Compensation

The estimated annual salary for this position is $85,000-$110,000, depending on experience and qualifications. Benefits include health, dental, and vision insurance, retirement plan, and paid time off.
