Job Summary: The Tulalip Tribes is seeking a skilled Healthcare Case Manager to join our Health Services team. As a key member of our interdisciplinary team, you will be responsible for the active case management of KIF Tulalip Health Clinic patients.

Key Responsibilities:

  • Collaborate with medical providers and interdisciplinary team members to establish individualized medical goals that optimize patient healthcare outcomes.
  • Coordinate multi-disciplinary care conferences for high-risk or complex patients who are referred into the case management process or who the Nurse Case Manager determines would benefit from the case management process.
  • Develop and implement comprehensive care plans that address the physical, emotional, and social needs of patients.
  • Provide education and support to patients and their families to promote health literacy and self-management skills.
  • Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.

Requirements:

  • Current and unrestricted Washington State RN license.
  • Negative TB Skin Test or chest x-ray prior to hire and annually as a condition of continued employment.
  • CPR/First Aid certificate.
  • HIPAA certification upon hire.
  • Must complete required Health Services credentialing process prior to start date.
  • Must abide by the Department's vaccination policy.
  • Certified as a Case Manager (CMC) or Certified Professional in Healthcare Management (CPHM) preferred.

Preferred Qualifications:

  • Three (3) years of general nursing experience, with three (3) months of employment as a clinical nurse at Tulalip Health Clinic or Two (2) years of general nursing experience, with one (1) year of clinical hospital/ambulatory experience.
  • Three (3) years of case management experience.
  • Three (3) years' experience working as a Registered Nurse (Employer will consider a combination of: One (1) year RN experience and Four (4) years LPN experience or Two (2) years RN experience and Two (2) years LPN experience as a substitute).
  • One (1) year work experience with electronic health record (EPIC preferred).
  • One (1) year work experience with phone triage as a professional nurse.
  • One (1) year work experience in handling acute care or emergent care as a professional nurse.

Terms of Employment: This is a regular Full-Time position.
