We are seeking a highly organized and detail-oriented Patient Discharge Coordinator to join our team at Johns Hopkins Healthcare. As a Patient Discharge Coordinator, you will play a critical role in ensuring a smooth transition of care for our patients.

Key Responsibilities
  • Support the discharge team by obtaining medical records and acquiring prior authorizations
  • Participate in rounds with physicians to coordinate care and orders after discharge
  • Provide exceptional patient service, ensuring a positive and professional experience for every patient
  • Expedite the use of automated scheduling, registration, and billing systems
Requirements
  • Minimum of High school diploma or equivalent required
  • Three years of previous experience in a healthcare setting
  • Experience working directly with physicians and experience with insurance and prior authorization
  • Experience with patient discharge and inpatient care
  • Familiarity with medical terminology and calendar management
  • Proficient in computer systems such as EPIC, SAP, and Thruput/bed board
