About the Role

**Job Summary:**

We are seeking a skilled and compassionate In-Home Healthcare Specialist to join our team at Focus Care. As a Per Diem Nurse Practitioner, you will be responsible for conducting in-person health risk assessments for Medicare, Medicaid, and Affordable Care Act members in their personal living spaces.

Key Responsibilities:

  • Conduct thorough and accurate health risk assessments in a home-based setting
  • Provide high-quality care and services to members, meeting health plan expectations and Focus Care quality standards
  • Communicate effectively with clinical leadership staff and maintain adequate documentation
  • Ensure confidentiality and compliance with HIPAA regulations
  • Make critical decisions while following Focus Care policies and procedures

Requirements:

  • Current Nurse Practitioner licensure in the state of New Hampshire
  • Board certification by the American Nurses Credentialing Center (ANCC) or the American Association of Nurse Practitioners (AANP)
  • Current BLS certification
  • Current TB Results (Skin or QuantiFERON accepted)
  • Enrolled in PECOS
  • Active CAQH Account
  • Valid unencumbered state drivers license

Preferred Qualifications:

  • Nurse Practitioner License in New Hampshire

Work Environment:

This is a per diem position, and you will be working on the road, conducting assessments in members' personal living spaces.
