Job Overview
As a key member of our healthcare team at RISE Life Services, the Licensed Practical Nurse (LPN) plays a vital role in delivering comprehensive care to individuals in a residential environment. Under the guidance of the Director of Nursing, the LPN is tasked with overseeing and addressing the healthcare needs of the individuals we support.

Key Responsibilities
The LPN will:
  • Collaborate with the Director of Nursing and Registered Nurse to ensure the highest standard of integrated health services.
  • Assist in executing nursing functions within the defined scope of practice as per established guidelines.
  • Collect and analyze data on individuals, including lab results and medical histories.
  • Support the development of individual care plans and identify specific healthcare needs.
  • Ensure adherence to physician orders and document observations and interventions as required.
  • Maintain effective communication with healthcare staff, physicians, and team members regarding individuals' medical statuses.
  • Oversee medical appointments and ensure follow-up actions are taken as necessary.
  • Provide training and support to residential staff to enhance the quality of care.
  • Respond to emergency situations and provide necessary coverage until relieved.

Qualifications
The ideal candidate will possess:
  • A valid NYS LPN License.
  • Experience working with individuals with developmental disabilities is preferred.
  • Knowledge of OPWDD regulations and the ability to manage multiple priorities effectively.
  • Strong communication and organizational skills.
  • A valid NYS Driver's License.
