Job Title: Licensed Practical Nurse

Sun River Health is seeking a dedicated and compassionate Licensed Practical Nurse to join our team at our Riverhead site.

Job Summary

The Licensed Practical Nurse will provide high-quality nursing care to patients, working in collaboration with the medical staff to support the delivery of comprehensive primary, preventative, and behavioral health services.

Responsibilities
  • Conduct patient interviews and maintain accurate medical records, including vital signs, health history, and medication information.
  • Perform in-house labs, collect specimens for outside labs, and ensure quality control in accordance with organizational protocols.
  • Complete and update assigned tasks and logs, and initiate follow-up as necessary.
  • Provide health teaching and education to patients, as directed by the physician or diagnosis.
  • Collect data on patients under the supervision of the Registered Nurse or provider.
Requirements
  • Valid New York State LPN License
  • CPR Certification
  • Infection Control Certification
  • High school diploma/GED
  • Preferred: Vocational/Technical School/Diploma Program in Nursing
