Sutter Health is seeking a highly skilled Ambulatory Clinical Pharmacist Lead to join our team. As a key member of our pharmacy department, you will be responsible for organizing and coordinating daily activities, ensuring compliance with federal and state laws, and serving as a liaison between staff, physicians, patients, and other hospital staff.
Key Responsibilities- Deliver pharmaceutical care through patient-centered clinical services, medication information, education, and medication preparation and distribution.
- Exercise sound judgment and developed clinical skills to provide input to and implement patient plans of care based on diagnosis.
- Gain confidence and cooperation from patients, families, and healthcare providers through competent patient assessment, monitoring, and care.
- Adhere to local, state, and federal regulations, codes, policies, and procedures to ensure patient privacy and safety.
- May be responsible for performing specific procedures and/or teaching duties.
- Doctorate degree from an accredited school of pharmacy.
- PHARMD license as a licensed pharmacist.
- 3 years of recent relevant experience.
- Full-time schedule with 40 hours per week.
- Variable days of the week with 8-hour shifts.
- As-needed weekend requirements.
