Job Summary

We are seeking a highly skilled Pediatric Care Coordinator to join our team at Community Health Solutions of America. As an On-Site Care Coordinator, you will play a critical role in providing care coordination, education, and outreach services to Medicaid members in the Medically Complex Children's Waiver program.

Key Responsibilities
  • Schedule appointments and conduct comprehensive assessments to determine Nursing Facility or Institutional level of care requirements.
  • Review and analyze LOC forms and medical records supplied by DHHS to qualify participation in the MCCW program.
  • Provide education to parents/guardians on caring for their child and arrange for appropriate education services as needed.
  • Develop and implement case-specific care plans and present them to providers for approval and/or revision.
  • Coordinate with Care Advocates to secure services approved on the care plan.
  • Maintain availability to family/guardians and providers to coordinate required services.
  • Develop disaster plans and identify availability of advanced directives, needs for ancillary services, specialty services, and community services.
  • Identify and address complaints and/or grievances.
Qualifications
  • A minimum of three (3) years of recent clinical pediatric experience is required.
  • Previous work experience with a Medicaid program is preferred.
  • Bilingual (Spanish) is preferred.
  • Ability to travel locally with a valid driver's license and automobile insurance.
  • Criminal background check as required by the state where business is conducted.
  • Negative TB test annually.
Education and Experience
  • A Bachelor's degree in a health-related field is strongly preferred.
  • A minimum of three (3) years of Pediatric Nursing, Pediatric Home Care experience is required.
Competencies
  • Strong organizational and interpersonal skills.
  • Excellent written and verbal communication skills.
  • Detail-oriented.
  • Ability to multi-task and work independently.
Certificates, Licenses, Registrations
  • Certified Case Management (CCM) Certification is required within four (4) years of employment.
  • Current unrestricted RN licensure in states where business is conducted.
  • Current BLS certification.
Computer Skills

Proficiency using software programs such as MS Word, ACCESS, PowerPoint, Excel, and Outlook.

Environmental Factors/Physical Demands

Work is performed in an office environment. While performing the duties of this job, the employee is regularly required to have the ability to maintain active customer and employee communication; access, input, and retrieve information from the computer system; enter alpha-numeric data into a computerized system often while listening on the telephone. May be subject to repetitive motion such as typing, data entry, and vision to monitor. May be subject to bending, reaching, kneeling, stooping, and lifting up to thirty (30) pounds.
