About the Job

We are seeking an experienced Hospice Registered Nurse (RN) / RN Case Manager to join our team. As a key member of our interdisciplinary team, you will provide direct patient care services, supportive care to patients' families, and empower caregivers through education and support.

Responsibilities
  • Report directly to the Administrator, Executive Director, or Patient Care Manager Senior.
  • Act as primary liaison between physician, patient/family, and hospice team.
  • Provide direct patient care services and supportive care to patients' families.
  • Empower patient's caregivers through education and support.
  • Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient.
Requirements

To be successful in this role, you must have:

  • A current RN license in the state of employment (or eligibility to obtain).
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology.
  • Demonstrated flexibility and composure while supporting others in a time of crisis.
