Job Overview

We are seeking a compassionate Registered Nurse to provide direct patient care services as a Hospice RN/ RN Case Manager. The ideal candidate will have a hospice heart, RN license in the state of employment, and 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology. The role involves acting as a primary liaison between physician, patient/family and hospice team, and managing and collaborating with an interdisciplinary group.
