Ambulatory Care Associate

We are seeking a highly skilled and compassionate Ambulatory Care Associate to join our team at Yale New Haven Health. As a key member of our healthcare team, you will play a vital role in providing exceptional patient-centered care and support to our patients and families.
Key Responsibilities- Provide Patient-Centered Care: Work collaboratively with patients, families, and healthcare providers to deliver high-quality, patient-centered care that meets the unique needs and preferences of each individual.
- Communicate Effectively: Utilize effective communication skills to interact with patients, families, and healthcare team members, fostering a culture of mutual respect and trust.
- Collaborate with the Healthcare Team: Function effectively within the interdisciplinary healthcare team, demonstrating open communication, mutual respect, and a commitment to teamwork and collaboration.
- Support Quality and Safety: Minimize the risk of harm to patients and providers through effective individual and team performance, adhering to high-reliability organization principles and behaviors.
- Utilize Informatics: Leverage information and technology to communicate effectively, mitigate errors, and support team decision-making.
- Develop Clinical Competencies: Demonstrate accountability for clinical competency in care delivery, staying up-to-date with the latest clinical practices and technologies.
- Education: High school diploma or equivalent, with a minimum of 1 year of experience as an Ambulatory Care Associate, PCA, MA, or CNA.
- Experience: Preferred 12-18 months of experience in an outpatient clinical setting or relevant patient care environment, with clinical competency.
- Licensure: Preferred current medical assisting credential (certifications or registration) acceptable to, and recognized by, the Connecticut Department of Public Health.
- Skills: Demonstrated ability to communicate effectively with patients, families, peers, and interdisciplinary team members, with a strong focus on flexibility, adaptability, and professionalism.
