Job Summary

We are seeking an experienced LVN to join our team as a Hospice LVN Per Diem. As a Hospice LVN, you will play a vital role in providing end-of-life care to our patients, working closely with our interdisciplinary team to ensure the highest quality of care and services.

Responsibilities
  • Coordinate with assigned patients to schedule visits at ordered frequencies.
  • Communicate with the hospice team regarding patient needs.
  • Provide education and support to patients and their families.
  • Monitor patient response to interventions and adjust care plans as needed.
  • Develop effective working relations and cooperation with hospice team throughout the entire case management process.
  • Take the extra initiative and interact with patients to advocate for their end-of-life goals.
  • Maintain accurate and timely documentation of patient progress notes.
  • Promote quality and cost-effective interventions and outcomes.
  • Adhere to professional standards as outlined by protocols, rules and regulations.
Requirements
  • Proven work experience in hospice care.
  • Effective communication skills.
  • Ability to work independently and as part of a team.
  • Excellent organizational and time management skills.
  • Strong clinical knowledge to be able to manage complex patient needs.
  • Compassionate and empathetic.
  • Current LVN license.
  • CPR Certified.
  • Valid driver's license and reliable transportation.
