Hospice Palliative Care RN Coordinator

We are seeking an experienced RN Case Manager to join our team in Colorado Springs. The successful candidate will be responsible for providing high-quality patient care, coordinating with healthcare professionals, and ensuring seamless care transitions.
Responsibilities- Develop and implement comprehensive care plans that address the unique needs of each patient
- Collaborate with patients, families, and healthcare professionals to ensure seamless care transitions and coordination
- Conduct thorough assessments of patients' physical, emotional, and spiritual needs
- Provide ongoing education and support to patients and families to promote optimal care and well-being
- Bachelor's degree in Nursing (BSN) preferred
- 1-2 years experience in hospice/palliative care/home health, medical surgical, public health, oncology, acute care or home health
- Current Colorado RN license and CPR certification
- Valid Colorado state driver's license and proof of auto insurance
