About the Role:

Matrix Medical Network is seeking a skilled Clinical Health Specialist to join our team. As a Clinical Health Specialist, you will conduct comprehensive health assessments in the comfort of patients' homes, providing unparalleled insight into their overall health and well-being. Key Responsibilities:

Conduct thorough health assessments in patients' homes, reviewing medical history, medication adherence, social environment, and other risk factors.
Collaborate with healthcare professionals to develop personalized care plans, ensuring patients receive the best possible outcomes.
Utilize Matrix's comprehensive health assessment tools to identify potential care gaps and provide targeted interventions.
Work collaboratively with patients, families, and caregivers to educate and empower them to manage their health effectively.

Requirements:

Master's degree in a relevant field or equivalent experience and licensure.
Current RN and NP/PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
Board certification in a Matrix-approved specialty.
Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
Strong computer skills and familiarity with EMR software.
