Job Summary

We are seeking a Hospice Intake Coordinator to join our team on a weekend schedule. As a key member of our team, you will be responsible for receiving referrals, providing explanations of benefits, and assisting with the scheduling of initial nursing assessments and admissions.

Responsibilities
  • Receive referrals by phone or in person and provide explanations of benefits to patients, families, and community contacts
  • Assist with finding alternative services when the patient is not a candidate for hospice
  • Oversee scheduling of initial nursing assessment and admission
  • Maintain admission packets and ensure that proper forms are available to admissions staff
  • Assist community outreach representatives with marketing activities to promote relations with referral sources
  • Adhere to work schedule and maintain good punctuality and attendance record
  • Provide telephone and clerical support to other staff members as time allows
Requirements
  • Be eighteen (18) years of age or older
  • At least one (1) year experience in homecare, hospice, and/or healthcare-related field preferred
  • Ability to complete necessary documentation appropriately, legibly, and timely
  • Ability to handle stressful and delicate situations
  • Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
  • Valid drivers license and proof of insurance are required
