Nurse Care Coordinator

The Nurse Care Coordinator will plan, organize, and direct hospice care, utilizing their expertise in nursing and community health education. This role involves building on the resources of the community to provide services that meet the needs of individuals and families within their homes and communities.
Key Responsibilities- Conduct comprehensive assessments of patients and families to determine hospice needs, providing a thorough physical assessment and history of current and previous illnesses.
- Provide professional nursing care by utilizing the nursing process, assessing and evaluating patient status, and participating in revising the plan of care as necessary.
- Develop a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions, involving the patient and family in the planning process.
- Initiate appropriate preventive and rehabilitative nursing procedures, administer medications and treatments as prescribed, and counsel patients and families on meeting nursing and related needs.
- Assist patients with activities of daily living, facilitate self-sufficiency and optional comfort care, and act as a Case Manager to coordinate patient care for assigned caseloads.
- Complete, maintain, and submit accurate and relevant clinical notes regarding patient condition and care given, recording pain/symptom management changes/outcomes as appropriate.
- Communicate with physicians regarding patient needs, report changes in patient condition, and obtain/receive physicians' orders as required.
- Teach patients and families/caregivers self-care techniques, provide medication, diet, and other instructions as ordered, and recognize opportunities for health counseling.
- Work in concert with the interdisciplinary group, providing and maintaining a safe environment for patients, and assisting patients and families/caregivers and other team members in providing continuity of care.
- Participate in on-call duties as defined by the on-call policy, ensuring arrangements for equipment and other necessary items and services are available.
- Supervise ancillary personnel, delegate responsibilities when required, and assume responsibility for personal growth and development, maintaining and upgrading professional knowledge and practice skills through continuing education and in-service classes.
- Fulfill the obligation of requested and/or accepted case assignments, actively participating in quality assessment performance improvement teams and activities.
