Primary Care Nurse Practitioner

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Conviva Care Centers. As a valued member of our primary care team, you will play a critical role in delivering high-quality, patient-centered care to our seniors.
Key Responsibilities- Evaluate and treat patients in accordance with established standards of care.
- Follow established levels of medical care and quality for patients, monitoring care through data reviews and chart analysis.
- Assist in coordinating patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care.
- Act as an active participant and leader in daily care team huddles.
- Collaborate with Regional Medical Director, Physician, and Center Administrator to set a tone of cooperation in practice, demonstrating a professional and approachable demeanor.
- Complete all medical record documentation in a timely manner, working with a quality-based coder to optimize coding specificity.
- Follow policy and protocol defined by Clinical Leadership.
- Meet with RMD to discuss quality of care, review outcome data, policy, procedure, and records issues.
- Participate in potential growth opportunities for new or existing services within the Center.
- Participate in the local primary care 'on-call' program of Conviva.
- Ensure personal compliance with licensing, certification, and accrediting bodies.
- Minimum of two to five years directly applicable experience preferred.
- Successful completion of a Nurse Practitioner program with Board Certification.
- Active, unrestricted Nurse Practitioner license in state of practice location.
- Excellent communication skills, written and verbal, with a high level of skill in interpersonal relationships and communications with all colleagues.
- Willingness and ability to learn/adapt to practice in a value-based care setting.
- Superior patient/customer service.
- Basic computer skills, including email and EMR.
- Appropriate certification to write prescriptions under the authority of a collaborating physician/medical director.
- Active and unrestricted DEA license.
- Medicare Provider Number/Medicaid Provider Number.
- Experience managing Medicare Advantage panel of patients with understanding of Best Practice in coordinated care environment in a value-based relationship environment.
- Knowledge of Medicare guidelines and coverage.
- Bilingual is a plus.
- Knowledge of HEDIS quality indicators.
