Job Summary

We are seeking a highly skilled and compassionate Assertive Community Treatment Nurse (RN) to join our team at Carolina Outreach. As a key member of our ACT team, you will provide clinical services to clients, including medication monitoring, psychopharmacologic and medical services, service coordination, crisis assessment and intervention, and individual counseling and psychotherapy.

Responsibilities
  • Provide clinical services to clients, including medication monitoring and documentation, psychopharmacologic and medical services, and individual counseling and psychotherapy.
  • Coordinate with medical staff of other providers, e.g., state and community inpatient facilities, to ensure comprehensive care for clients.
  • Assist in identifying, planning, scheduling, and implementing services for clients, including crisis phone and on-call duties.
  • Develop transition plans for clients moving in and out of services, and provide written documentation as required.
Requirements

To be successful in this role, you will need:

  • A current RN license in the state of North Carolina.
  • Experience in community-based psychiatric nursing, preferably in an Assertive Community Treatment (ACT) setting.
  • Strong clinical skills, including medication management and individual counseling.
  • Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare providers.
