Responsibilities
As an After-Hours Hospice RN, you will be responsible for conducting comprehensive assessments and providing timely delivery of hospice care to patients and their families after regular business hours. This includes triaging patient needs over the phone, responding to crisis calls, and making initial nursing evaluations to determine eligibility for hospice services. You will also perform visits, including admission, death, recertification, and routine visits, while identifying and addressing patients' physical, psychosocial, emotional, and environmental needs. Your documentation skills will be essential in capturing patient problems, goals, interventions, and responses to care.

Requirements
To be successful in this role, you must have at least one year of clinical experience as a Registered Nurse and hold a valid RN licensure in the state of practice. Current CPR certification and a dependable vehicle or access to public transportation are also required. If you possess strong communication and interpersonal skills, a passion for patient-centered care, and the ability to work independently with minimal supervision, we encourage you to apply.

Compensation
The estimated salary for this position is $80,000-$110,000 per year, based on experience and location.
