Job Summary

The LPN will be responsible for delivering direct nursing care to patients with acute and chronic illnesses, as well as recording clinical and progress notes. The successful candidate will work under the supervision of a Registered Nurse and will be expected to maintain the highest standards of patient care.

Responsibilities
  • Provide high-quality clinical services within the scope of practice, as defined by state laws governing nursing practice.
  • Document patient visits thoroughly and completely, adhering to regulatory and payer requirements.
  • Participate in the agency's care coordination process, including timely reporting of patient needs and changes in condition.
  • Ensure confidentiality of patient health information, as per agency policy.
Qualifications
  • Current LPN licensure in the state of practice.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification.
