Position Overview

As the Admissions Nurse, you will take the lead in managing the admission process, which encompasses assessing, planning, implementing, and evaluating patient care strategies. Your responsibilities will include welcoming new patients and facilitating their transition into our facility. You will provide a Welcome Packet, address inquiries, and initiate the C.A.R.E.S. process. Your role will involve coordinating communication among family members, nursing home staff, social workers, and the healthcare team throughout the admission journey. You will also be responsible for documenting the initial care plan in the electronic medical record (EMR), conducting comprehensive assessments, and ensuring that all necessary treatments and orders are established. Additionally, you will maintain a weekly wound log and implement preventative measures as required (e.g., skin care, fall risk assessments, elopement precautions).

Furthermore, you will manage all essential admission documentation, including the initial care plan, medication lists, admission forms, physician orders with appropriate dates and signatures, code status documentation, and other mandatory records. It will be crucial to verify orders with the attending physician and to gather and input the patient's current immunization status into the EMR. You will also have the opportunity to offer, administer, and document immunizations as needed. Familiarity with agency policies and regulatory requirements is essential, and you may be assigned additional responsibilities as necessary.

Qualifications
  • Graduate of an accredited nursing program
  • Bachelor's and/or Master's degree is preferred
  • Current nursing license in good standing within the state of employment
  • A minimum of two years of diverse nursing experience, with at least one year in a post-acute care setting preferred
  • Proficient in administrative tasks, with strong written and verbal communication skills
  • Ability to assess and respond effectively to the needs of patients and their families in various environments
  • Demonstrated nursing skills and practice
  • Capacity to manage family emotional stress and respect individual lifestyles
  • Physical ability to lift, transfer, and assist patients safely
  • Critical thinking skills and proficiency in psychosocial assessments
  • Manual dexterity and adeptness in handling patients and medical equipment
