Job Summary:We are looking for a dedicated and experienced Registered Nurse to join our team as an Admissions Registered Nurse. In this role, you will be responsible for completing admissions and/or visits for our Home Care team. You will work independently and collaboratively with our interdisciplinary team to provide patient-centered care and ensure the best possible outcomes for our patients.
Responsibilities:- Complete admissions and/or visits for our Home Care team
- Work independently and collaboratively with our interdisciplinary team to provide patient-centered care
- Ensure a seamless transition for our patients
- Provide high-quality patient care and education
- Collaborate with our team to develop and implement individualized care plans
- Graduate of an accredited school for RN and licensed to practice as a Registered Nurse in the state of Minnesota
- Minimum of twelve (12) months experience working in an acute care or home health environment
- Evidence of current CPR certification
- Experience in Home Care to include OASIS; minimum one (1) year experience preferred
- Able to travel within a geographic area serviced by the office
- Reliable transportation, valid drivers license, and proof of auto insurance is required
- Annual Salary Range: $76,960 - $89,440
- $6,000 Hiring Bonus
- Mileage Reimbursement / Company Car Program
