Transforming Healthcare from Home
We are seeking a skilled Nurse Practitioner to join our HouseCalls team, providing annual clinical assessments to patients in the comfort of their own homes. This vital preventive care helps identify and reduce health risks, coordinating appropriate follow-up care to improve overall well-being.
Key Responsibilities:
- Conduct in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams.
- Perform evidence-based practice screenings, including point-of-care testing as necessary.
- Identify diagnoses for care management and active medical management in the furtherance of treatment.
- Communicate findings to inform members' primary care physicians of potential gaps in care.
- Intervene in urgent and emergent situations as needed.
- Educate members on disease processes, medication, and compliance.
Requirements:
- Active NP license or ability to obtain by start date, with unencumbered licensure.
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or ability to obtain national certification and/or NP license in the state of assignment by start date.
- Ability to complete physical requirements of the job, including lifting a 30-pound bag, navigating stairs, and various dwelling conditions.
- Reliable transportation for home visit assessments.
Preferred Qualifications:
- May be requested to obtain additional licensure in other geographic areas.
- 1+ years of clinical experience in the highest level of education, clinical setting.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Home health care or home visit experience.
- Ability to travel throughout the state, beyond the assigned region, up to 5% of the time, with advanced notice and potential for additional compensation incentives.
- Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages.
