Job Title: Case Manager

We are seeking a highly motivated and experienced Case Manager to join our team at the Administrative Office of the Courts. As a Case Manager, you will be responsible for providing support and guidance to individuals with substance use disorder and mental health issues.

Key Responsibilities:
  • Maintain a caseload of participants and conduct regular assessments to identify their needs and develop personalized plans.
  • Provide referrals to community resources, treatment programs, and other services to support participants' recovery and well-being.
  • Collaborate with the Specialty Courts team and judges to ensure participants' progress and provide updates on their status.
  • Conduct home visits and administer drug tests as needed.
  • Maintain accurate records and reports on participants' progress and outcomes.
Requirements:
  • Associate's degree in a related field or equivalent experience.
  • At least one year of experience in a related field, such as social work, counseling, or case management.
  • Strong communication and interpersonal skills, with the ability to work with diverse populations.
  • Ability to work in an office setting and conduct home visits, with occasional statewide and overnight travel.
