Job Summary

The Nurse Care Coordinator is a seasoned nursing professional with a focus on community health education and experience. This role involves planning, organizing, and directing home care services to meet the needs of individuals and families in their homes and communities.

Responsibilities

  • Develop and implement personalized care plans for patients
  • Collaborate with healthcare teams to ensure seamless care transitions
  • Conduct home visits to assess patient needs and provide education
  • Manage and coordinate home health care services
  • Communicate effectively with patients, families, and healthcare providers

Requirements

  • Current and unencumbered RN license in the state of employment
  • Bachelor's degree in Nursing from an accredited program (preferred)
  • 1-2 years of recent acute care experience in an institutional setting
  • 1 year of home health care experience (preferred)
  • Direct supervisory experience of LVNs and NAs (preferred)
  • Valid CPR certification
  • Working knowledge of OASIS documentation requirements (preferred)
  • Excellent observation, verbal, and written communication skills
  • Basic math skills and nursing skills per competency checklist
  • Access to reliable transportation
