Medical Professional for Patient Care and Population Health

We are seeking a dedicated Medical Professional to join our team at Mass General Brigham. This role is ideal for an individual with experience in patient care and population health.
About the Role:
- This position involves providing direct and indirect patient care using evidence-based practice and the nursing process (assessment, planning, implementation, and evaluation).
- The successful candidate will work collaboratively with multidisciplinary teams to provide comprehensive care in accordance with professional standards and regulations.
Responsibilities:
- Deliver high-quality patient care, prioritizing physical, emotional, and social needs.
- Collaborate with healthcare professionals to develop and implement individualized care plans.
- Maintain accurate and timely documentation of patient progress and outcomes.
- Participate in performance improvement activities to enhance quality and efficiency of care.
Requirements:
- Degree in Nursing from an accredited institution.
- Minimum 1 year of Med/Surg experience.
- Strong interpersonal and communication skills.
- Certifications and/or Registrations: CPR Training required within first 3 months of employment.
