Key Responsibilities
  • Assist with phone calls from field staff, physician offices, referral sources, and patients, taking messages and orders as needed.
  • Monitor the status of incoming faxes and report information to appropriate personnel.
  • Coordinate communication and faxes of applicable documents to physician offices.
  • Monitor patient status in the hospital to ensure adequate care coordination.
  • Assist the Patient Care Manager with managing software system reports for effective care coordination and utilization management.
Requirements
  • Current LVN licensure in the state of practice.
  • Current CPR certification.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle.
