We are seeking a compassionate and skilled Physical Therapist to join our Home Health Admissions team at Jefferson Health at Home by BAYADA. As a Physical Therapist, you will play a vital role in providing high-quality patient care and ensuring the best possible outcomes for our patients.

Key Responsibilities
  • Assess and Develop Care Plans: Conduct thorough assessments of patients' physical needs and develop personalized care plans to address their unique requirements.
  • Provide Physical Therapy Services: Deliver high-quality physical therapy services to patients in their homes, including manual therapeutic exercises, gait training, balance, and other interventions.
  • Collaborate with Healthcare Team: Work closely with nurses, physicians, and other healthcare professionals to ensure seamless care coordination and effective patient outcomes.
  • Communicate Effectively: Maintain open and transparent communication with patients, families, and healthcare team members to ensure informed decision-making and optimal care.
Requirements
  • Active Physical Therapy License: Possess an active and current license to practice as a Physical Therapist in the state of PA.
  • Home Healthcare Experience: Previous experience in home healthcare is welcomed, but not required. We will provide training and support to ensure your success.
  • Travel Requirements: Ability to travel to patient homes as assigned.
  • Strong Communication Skills: Possess excellent assessment skills, active listening skills, and strong written and verbal communication.
