Street Outreach Coordinator

Central City Concern is seeking a highly motivated and compassionate Street Outreach Screening/Intake Coordinator to join our team. As a key member of our outreach team, you will be responsible for providing screening, outreach, and service brokerage to participants referred to our LEAD program. You will work closely with community resources to broker services and provide access to address client needs, including substance abuse treatment, medication management, physical health treatment, mental health treatment, benefits and entitlements, appropriate and affordable housing, employment and educational opportunities, and supports for basic needs.
Responsibilities- Provide street outreach to engage individuals who have been referred to LEAD with the purpose of facilitating completion of intake process.
- Develop and maintain positive, collaborative relationships with all LEAD partners and other service providers to best serve LEAD participants.
- Complete initial screening and intake with all new referrals.
- Assess and support accessing services for emergent/immediate client needs, including medical care, shelter, food, and clothing.
- Provide street outreach and short-term case management services to engage individuals who have been referred to LEAD.
- Manage, coordinate, and facilitate the assignment/hand off of new referrals to a case manager, peer, or housing.
- Provide the assigned case managers with completed screening paperwork and relevant information.
- Work in tandem with case managers to provide street outreach to engage participants when needed.
- Assist Program Supervisor to maintain documents listing all LEAD participants.
- Attend all LEAD Program staff meetings and LEAD Operations Team meetings.
- Maintain all records as required.
- HSD/GED required.
- Education and experience appropriate for this role, as demonstrated by one of the following: QMHA-R, CADC-R, Bachelor's Degree in a service-related field.
- Experience working with homeless individuals and providing street outreach, or outreach-based community social service or healthcare environment preferred.
- An undergraduate degree in a behavioral health field preferred.
- Lived experience preferred.
- Bi-lingual preferred.
- Must have current CPR certification prior to start date.
