Job Description

We are seeking an experienced Registered Nurse to join our team as a Nurse Care Specialist. In this role, you will be responsible for providing high-quality patient care and collaborating with our multidisciplinary team to achieve optimal outcomes.

Your Key Responsibilities

  • Patient Assessment: Conduct thorough assessments of patients' physical and emotional needs, identifying opportunities for improvement and developing individualized care plans.
  • Care Coordination: Work closely with physicians, nurses, and other healthcare professionals to ensure seamless care transitions and effective communication.
  • Nursing Practice: Apply evidence-based nursing practices to provide safe and compassionate care, adhering to professional standards and guidelines.

Requirements

  • Licensure: Current licensure as a Registered Nurse in the State of Vermont.
  • Certifications: Basic Life Support (BLS) certification through the American Heart Association and unit-specific certifications as required.
  • Education: Graduate of an accredited school of nursing, Bachelor of Science in Nursing (BSN) preferred.
