Job Summary

We are seeking a skilled RN Case Manager to join our team at Ascension Saint Agnes in Baltimore, Maryland. As a RN Case Manager, you will play a critical role in coordinating the overall interdisciplinary plan of care for patients, from admission to discharge.

Responsibilities
  • Develop and implement comprehensive care plans that integrate patient preferences and values.
  • Monitor patient medical necessity and level of care through ongoing assessments and evaluations.
  • Advocate for resources and removal of barriers to ensure effective care delivery.
  • Maintain open communication with supervisors and care transition team members to ensure seamless care transitions.
  • Provide education and support to patients and families to promote self-care and independence.
Requirements
  • Registered Nurse credentialed from the Maryland Board of Nursing required.
  • BLS Provider certification preferred.
  • Case Manager certification preferred.
