Job Summary

We are seeking a compassionate and skilled Registered Nurse to join our interdisciplinary team. As an RN Resource, you will play a vital role in delivering patient-centered care and making a meaningful difference in the lives of our patients and their families.

Key Responsibilities
  • Conduct thorough patient assessments to identify physical, emotional, and spiritual needs
  • Develop and implement individualized plans of care in collaboration with the interdisciplinary team
  • Provide education and support to patients and families on end-of-life care, pain management, and symptom control
  • Collaborate with healthcare professionals to ensure seamless care transitions and coordination
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Requirements
  • Current, unrestricted Registered Nursing License in the state of employment
  • One year of nursing experience, preferred
  • Valid driver's license, auto insurance, and access to a reliable vehicle
  • Ability to develop and maintain knowledge of hospice and end-of-life care
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced, dynamic environment
