Position Overview:
The Advanced Nurse Practitioner role at our healthcare organization is centered on delivering exceptional primary care to Medicare beneficiaries, with a strong focus on preventive healthcare and tailored treatment strategies. You will autonomously oversee patient care while working in collaboration with a diverse healthcare team to provide comprehensive and effective medical services. Your duties will encompass performing routine evaluations, coordinating care with specialists, and educating patients about their health and wellness.

Key Responsibilities:
  • Provide outstanding primary care services
  • Conduct office visits for both routine and urgent health issues
  • Carry out wellness examinations and health risk evaluations
  • Coordinate care with other healthcare providers and facilities
  • Spearhead population health initiatives
  • Assist with telephone triage and patient outreach efforts
  • Educate patients regarding their health conditions and treatment options
  • Engage in promotional activities
  • Perform home visits when necessary
  • Collaborate effectively with the healthcare team
Essential Qualifications:
  • Advanced nursing degree or Nurse Practitioner certification
  • National certification in a relevant specialty area
  • Active state Nurse Practitioner license and DEA registration
  • Authorization to work in the United States
  • Strong dedication to patient-centered care
