Job DescriptionWe are seeking a skilled Nurse Case Manager to join our team at Rehabilitation Care Coordination. As a key member of our team, you will play a vital role in advocating for the welfare of our patients and serving as a liaison between the client, their family, financial advocate, and any healthcare provider.

A successful Nurse Case Manager will have a strong background in nursing, with a focus on rehabilitation and case management. You will be responsible for reviewing and interpreting medical records, diagnoses, treatment options, and findings for patients. You will assess, plan, implement, coordinate, and evaluate patient care needs, ensuring that our patients receive the best possible care.

As a Nurse Case Manager, you will be responsible for coordinating resources and benefits available through various government, community, local educational institutions, and other healthcare providers. You will establish strong relationships with patients and their families, as well as with healthcare providers and other stakeholders.

We are looking for a highly motivated and organized individual who is able to work independently and as part of a team. You will need to have excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers.

Requirements:

  • California Nursing License
  • Bachelor's degree in Nursing
  • At least 3 years of acute nursing experience, with rehabilitation experience preferred
  • Ability to drive to appointments and patient visits
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
