Job Description

CAMBA is a community-based organization dedicated to empowering families and promoting children's well-being. We are seeking a skilled Case Planner to join our School-Based Early Support Program team.

Key Responsibilities:

  • Conduct outreach activities to engage families and discuss program offerings and expectations.
  • Collaborate with referral sources, including schools and community-based organizations, to gain a comprehensive understanding of each family's unique strengths, needs, and priorities.
  • Develop individualized support plans in partnership with families, based on comprehensive assessments that identify families' strengths, needs, and service requirements.
  • Manage a caseload of 12 clients effectively and maintain accurate documentation.
  • Prepare reports and participate in program evaluation.

Requirements:

  • A Bachelor of Arts (BA), Bachelor of Science (BS), or Bachelor of Social Work (BSW) degree.
  • Minimum of two years of experience working with families and children in a community setting.
  • Strong interpersonal and communication skills, with the ability to convey respect, compassion, and empathy to clients.
