Position Summary:
The Bilingual Health Coach plays a vital role within our onsite primary care team, assisting patients in improving their health by equipping them with the necessary tools and motivation to make healthier lifestyle choices. This position serves as the initial point of contact for patients regarding health and wellness inquiries, both within the clinic and in broader contexts.
Key Responsibilities:
- Establish enduring relationships with patients visiting the wellness center.
- Facilitate both group and individual diabetes self-management education sessions based on evidence-based protocols.
- Measure patients' vital signs and accurately document their visits with healthcare providers.
- Reinforce medical recommendations and assist patients in developing actionable plans to follow through.
- Engage in outreach initiatives, both onsite and offsite, to encourage patient participation in health and wellness programs.
- Deliver outpatient diabetes education and self-management training to individuals with type 1, type 2, and gestational diabetes, covering essential topics such as hyperglycemia, hypoglycemia, DKA, and HHNK.
- Design, implement, and oversee diabetes education programs for adults, children, and their families.
- Instruct patients on the evidence-based Standards of Care for diabetes, obesity, hypertension, and other chronic conditions.
- Monitor and assess patient responses to therapeutic interventions in collaboration with healthcare professionals.
- Identify opportunities for enhancing patient care and present recommendations to leadership.
- Travel may be necessary to various locations for mobile clinic services.
- This role may involve visiting remote areas as part of our mobile clinic operations.
Qualifications:
- High School Diploma or GED.
- A minimum of 2 years of experience in nutritional services.
- Fluency in Spanish, both written and verbal, is required.
- Exceptional communication skills, both oral and written.
Skills and Competencies:
- Strong knowledge of community wellness, nutrition, and physical fitness.
- Excellent communication and customer service abilities.
- Effective time management and organizational skills.
- Proficient in Microsoft Office Suite, including Word, Excel, PowerPoint, and Outlook.
- Ability to work independently with minimal supervision.
- Demonstrated proficiency in oral and written communication.
Compensation: $27.00 per hour.
