Job Description

The Clinical Care Coordinator plays a vital role in ensuring seamless patient care at Bozeman Health. As a key member of our team, you will collaborate with healthcare providers and departments to coordinate services and guarantee quality care for patients.

This position requires a skilled nurse who can assess, evaluate, and implement care delivered to patients, facilitating a smooth transition from clinic to procedural and outpatient treatment areas. The successful candidate will work closely with the multidisciplinary team to schedule appointments, diagnostic testing, and procedures, expediting the plan of care and promoting continuity of care.

In addition to these responsibilities, the Clinical Care Coordinator will also be responsible for planning and implementing patient and family education.
