Patient Care Manager Opportunity

We are seeking a dedicated and compassionate Patient Care Manager to join our team at MASC Medical Recruitment Firm. As a Patient Care Manager, you will play a vital role in enhancing the quality of patient management, maximizing patient satisfaction, and promoting cost-effectiveness.

Key Responsibilities:

  • Assist patients with various financial and economic needs
  • Update the team with important issues related to patient needs to ensure overall health
  • Stay up-to-date with knowledge in this space

Requirements:

  • Bachelor's Degree (preferred) Associates Degree (required)
  • 1+ Years in outpatient Behavioral Health field and/or Social Support field
  • 2+ Years in direct care management position conducting biopsychosocial assessments and care plan development (preferred)
  • Understanding of how to navigate Federal and State programs, including disability, food stamps, and IHSS
  • Comfortable managing more than 80 patients
  • Bilingual (preferred)

Benefits:

  • Compensation: $60,000 - $70,000 annually
  • 3 weeks (2 weeks + 6-7 federal holidays) paid vacation and 401K
  • Health insurance covered by the employer
