SalaryThe estimated annual salary for this role is $70,000 - $90,000, depending on experience and qualifications.
Job DescriptionThis is a rewarding opportunity to join our team as a Compassionate Care Specialist - Home Health & Hospice. As a key member of our healthcare team, you will provide comprehensive social work services to patients in their homes, promoting their overall well-being and quality of life.
ResponsibilitiesKey responsibilities of this role include:
- Assessing the psychosocial status of clients related to their illness and environment
- Developing and implementing personalized care plans to address the unique needs of each client
- Providing education and support to clients and their families/caregivers
- Maintaining accurate and up-to-date clinical records
- Collaborating with healthcare teams to ensure seamless care coordination
To be successful in this role, you will possess:
- A Bachelor's degree in Social Work or a related field
- Current licensure as a Social Worker in Minnesota and North Dakota
- At least one year of experience in healthcare, preferably in home health and hospice
- Excellent verbal and written communication skills, as well as strong organizational and time management skills
