Job Title: Clinical Diabetes Educator - Registered Nurse Care Manager

This is a grant-funded position that requires a Registered Nurse with experience in diabetes education and care. The successful candidate will provide comprehensive diabetes education and care to patients with Type 1 diabetes, Type 2 diabetes, gestational diabetes, and those at high risk of developing diabetes in a clinic setting.



The educator will be responsible for ordering labs for patients, documenting education in the electronic health record, and working closely with village clinic and hospital staff to promote diabetes standards of care and inform them of new advances in diabetes care.



The educator will also participate in program outreach activities such as monthly Diabetes Support Groups and radio shows, media campaigns, and special events.



Requirements include a minimum of 2 years of experience in a clinical health setting, a valid license in good standing in the State of Alaska as a Registered Professional Nurse, and the ability to obtain valid CDCES certification within 2 years.
