Job Summary

We are seeking a skilled Case Manager to join our team at The Methodist Home of Kentucky, Inc. This role will involve working with youth who are on home incarceration or home supervision, providing support and guidance to ensure successful completion of court mandates.

Key Responsibilities
  1. Provide regular face-to-face contacts and daily telephone calls to monitor youth progress and ensure compliance with court requirements.
  2. Collaborate with Department of Juvenile Justice/Detention Alternatives Coordinators (DAC) to receive referrals and process intakes.
  3. Maintain accurate records and documentation, including weekly progress reports and monthly billings.
  4. Attend professional meetings, workshops, and training sessions to stay up-to-date on best practices and industry developments.
  5. Develop and maintain relationships with community workers and DAC to ensure effective case planning and coordination.
Requirements
  1. High school diploma or equivalent required; previous case management or juvenile services experience preferred.
  2. Valid Kentucky driver's license and ability to pass an intensive background investigation.
  3. Reliable transportation for extensive travel.
