The Clinical Care Nurse Coordinator will serve as the primary care manager for a group of members with intricate medical and behavioral requirements. The focus of care coordination will be on addressing members' medical, behavioral, and socioeconomic needs to encourage appropriate service utilization and elevate the quality of care.
Key Responsibilities:
Conduct face-to-face and/or telephonic interactions with members to perform comprehensive needs assessments, including evaluations of medical, behavioral, functional, cultural, and socioeconomic factors.Design and execute individualized care plans that address needs such as chronic health condition management, health promotion, wellness, social determinants of health, medication oversight, and member safety, all in accordance with evidence-based practices.Collaborate with internal care teams, healthcare providers, and community resources to implement care plans effectively.Offer education and coaching to empower members in self-managing their care needs and making lifestyle adjustments to enhance their health.Facilitate proactive discharge planning and oversee care transitions following emergency room visits, inpatient stays, or skilled nursing facility admissions.Advocate for members and their families to ensure their needs and preferences are fully represented and supported by the healthcare team.We recognize and reward your performance in an environment that will challenge you and provide clear guidance on achieving success in your role, as well as opportunities for growth into other positions of interest.
Required Qualifications:
Current, unrestricted independent licensure as a Registered Nurse.Minimum of 2 years of clinical experience.At least 1 year of proficiency with MS Office applications, including Word, Excel, and Outlook.Access to reliable transportation and the ability to travel within the designated territory to meet with members and providers.Preferred Qualifications:
Bachelor of Science in Nursing (BSN) or a higher degree in a clinical field.Certification in Case Management (CCM).1+ years of experience in community case management for individuals with complex needs.Experience in team-based care settings.Background in managed care environments.Physical Requirements:
Ability to transition between office and field locations multiple times throughout the day.Capability to navigate various locations and terrains to visit members and providers.Ability to transport necessary equipment for field visits (e.g., laptop, stethoscope).Capacity to remain stationary for extended periods to complete computer or tablet tasks.Compensation: The hourly wage for this position ranges from $28.03 to $54.95, based on various factors including local labor markets, education, work experience, and certifications.
