Job Title: Licensed Practical Nurse (LPN)

Aveanna Healthcare is seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in the home setting. As a LPN, you will be responsible for delivering high-quality patient care, documenting patient information, and collaborating with the healthcare team to ensure optimal patient outcomes.

Job Summary:

The Licensed Practical Nurse (LPN) will provide one-to-one patient care in a home environment, executing physicians' prescribed plans of care and documenting care provided in the system of record. The LPN will also administer prescribed medication, treatments, and therapies, as well as perform patient assessments and coordinate care.

Responsibilities:
  • Provide one-to-one patient care in a home environment
  • Execute physicians' prescribed plans of care and document care provided in the system of record
  • Administer prescribed medication, treatments, and therapies
  • Perform patient assessments and coordinate care
  • Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner
Requirements:
  • Graduate of an accredited school of nursing
  • Current, unrestricted state license as a Licensed Nurse in the state of practice
  • Current CPR certification
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures
Preferred Qualifications:
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
  • Home health experience
