Job Summary:

  • Evaluate and admit patients to home care, developing personalized plans of care and providing compassionate support according to each patient's needs.">
  • Serve as a case manager, making necessary adjustments to patient plans based on symptom changes and evolving care requirements.">
  • Review and plan patient care and discharge during scheduled case conferences, ensuring seamless transitions and accurate documentation.">
  • Maintain open communication with patients, caregivers, physicians, and community healthcare partners, fostering collaborative relationships and promoting quality care.">
  • Strive to optimize cost-effectiveness by efficiently managing mileage, supplies, and visit scheduling.">
  • Complete comprehensive documentation related to patient interactions, assessments, service outcomes, and recertifications.">
  • Ensure compliance with policies regarding resumption of care and discharge procedures.">
  • Stay current with home care regulatory guidelines, upholding standards of excellence.">
  • Provide on-call coverage for both home care and hospice services as assigned, demonstrating flexibility and adaptability.">
  • Foster a culture of teamwork and exceptional patient experiences, emphasizing patient safety and quality.">

Qualifications:

To be successful in this role, you will require:

  • An Associate's degree in nursing and a valid license to practice in Alabama.">
  • A minimum of 2 years' experience in a hospital setting, preferably in med-surg, critical care, or ER environments.">
  • Prior experience in home care or hospice settings is highly valued.">
  • Demonstrated expertise in critical thinking, organizational skills, and independent decision-making.">
  • The ability to work independently and prioritize tasks effectively.">
