Job Title: Case Manager

Job Summary:

Lantern Community Services is seeking an experienced Case Manager to join our team in New York, NY. The ideal candidate will have a strong clinical background and experience working with individuals impacted by mental health illness, substance abuse, and other health issues.

Key Responsibilities:

  • Assess client needs and develop individualized plans to achieve stabilization and re-housing.
  • Link clients to services, including benefits and entitlements, case assistance, and group programming.
  • Provide individual counseling and support, advocacy, resources, and referrals to clients.
  • Monitor progress and document client contacts.
  • Develop safety plans for residents and work with the team to deescalate clients and manage crisis situations.

Requirements:

  • BA/BS required, preferably in a related field. Master's degree preferred.
  • One year of experience working with single adults impacted by poverty, chronic homelessness, mental illness, substance abuse, and/or medical issues required.
  • Must be fluent in Spanish.
  • Highly organized and able to manage a diverse case load of clients with diverse needs.
  • Able to work well under pressure, including in crisis situations, and be flexible with work schedule when necessary.

Work Environment:

This job operates in a supportive housing site in New York, NY, and works with a population who may suffer from mental illness, substance abuse, disability, HIV/AIDS, and/or other health issues.

Physical Demands:

While performing the duties of this job, the employee is regularly required to stand, walk, sit, stoop, climb stairs, and/or kneel. Occasionally, the employee will have to lift and/or move up to 10 pounds.
