Adult Support Navigator

We are seeking a dedicated Community Relations Coordinator to join our team, focusing on building and maintaining positive relationships within the community and sharing them with our families and clients. The ideal candidate will provide personalized support to help adults with developmental disabilities thrive in their home, community, or workplace.
Key Job Responsibilities:- Identify and gather essential resources for clients, prioritizing their needs.
- Communicate effectively with clients, parents, and providers about policies, procedures, and available resources.
- Develop and maintain a strong network of community connections, expanding opportunities for clients.
- Maintain accurate and up-to-date database records, facilitating easy access to critical information.
- Navigate complex systems, such as Medi-cal, SSI, and other essential services, to ensure clients receive the necessary support.
A minimum of two years of experience in case management, social services, or a related field is required. Proficiency in Microsoft Office, strong organizational skills, and effective communication are essential for this role.
