About the Role

We are looking for a Physical Therapist who is passionate about delivering exceptional care and is committed to helping our clients achieve their goals. As a Home Health Physical Therapist, you will work with clients in their homes, developing and implementing individualized care plans that address their unique needs and abilities.

Responsibilities
  • Conduct thorough assessments of clients' physical abilities and develop personalized care plans
  • Provide manual therapeutic exercises, gait training, and other interventions to help clients achieve their goals
  • Continuously assess and revise care plans to ensure optimal outcomes
  • Educate clients and their families on rehabilitative care and activities necessary for independent living
  • Accurately document observations, interventions, and evaluations

Requirements
  • Current Physical Therapist license in HI state
  • Minimum of one year of recent work experience as a Physical Therapist
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
  • Solid computer skills and prior experience with electronic medical records (EMR) preferred
